What causes Rbbb and LAFB?
RBBB + LAFB is the most common of the two patterns. This is due to a single coronary artery blood supply (LAD) to the anterior fascicle. RBBB + LPFB is less common due to a dual blood supply (right and left circumflex arteries), and this combination may be associated with more extensive underlying cardiac pathology.

Background. Right bundle branch block (RBBB) and left anterior fascicular block (LAFB) are very common findings. In the presence of second degree atrio-ventricular (AV) block, this condition can be life threatening and mandates emergent treatment.
What is a right bundle branch block with left anterior fascicular block?
When we have right bundle branch block with either left anterior fascicular block (common) or left posterior fascicular block (less common) we call it a bifascicular block. The problem with bifascicular block is that the heart’s electrical conduction system is down to one fascicle.

In people with known or suspected heart disease, right bundle branch block is associated with a greater risk of death, especially after a heart attack. Some people with right bundle branch block may ultimately need a permanent pacemaker, but this is rare.

In most people, bundle branch block doesn’t cause symptoms. Some people with the condition don’t know they have bundle branch block. Rarely, signs and symptoms may include fainting (syncope) or feeling as if you’re going to faint (presyncope).
Is left anterior fascicular block considered heart disease?

What is the treatment for LAFB?
There currently is no treatment for people with LAFB.
Does RBBB need treatment?
RBBB doesn’t always require treatment, especially if you don’t have any underlying heart conditions. If you do have another heart condition, your doctor might suggest treating the underlying cause. If you have RBBB due to a heart attack with other electrical system conditions, for example, you may need a pacemaker.
